MOBE Guides bring encouragement, direction, and accountability—and enable people to make lasting change. It starts with listening and helping to set goals. It continues with regular phone calls and app-based chats, and by sharing hand-picked articles, recipes, and health tips.
After a thorough medication review, Pharmacists work together with participants and their health care providers to build a medication approach that supports health goals. From there, periodic check-ins and chats keep things on track.
Our health app is the on-the-go resource for information, motivation, and communication. It’s a convenient way to schedule a person-to-person check-in, chat with a Guide or Pharmacist, dig into health resources, track goals, and much more.
